Before your visit:
Parking: We do not have a parking lot though metered street parking is usually easy to find. There is a paid lot at 2610 1st Ave. (on 1st between Vine and Cedar) 1.5 blocks away.
Arrival: New patients please arrive 15 minutes early to check in. Bring your photo ID and insurance card. Return patients please check in 5-10 minutes prior to the appointment start time.
Weekend massage: We do not have reception on the weekends. The door may be locked more than 10 minutes prior to your scheduled time.
Intake forms: Please see links below to our paperwork. We will send you instructions if we need you to complete any of these forms. If you have been an active patient in the last year, you do not need to do any new paperwork unless you have a new car accident or work injury.
Misc: Our office is not a fragrance-free facility, though we ask that staff and visitors keep fragrances to a minimum for sensitive individuals. Please inform us ahead of time if you have any specific needs that we should be aware of.
Clinic Intake Forms
If we need you to complete any forms prior to your visit, we will send you the appropriate link and instructions. If you have been an active patient in the last year, no forms are needed unless your information changed or you were in a new accident.

New Patient Intake Form
We are accepting new patients! The first step is to complete the New Patient Intake Forms so we can create your patient profile. We can offer you possible appointment times, but need to have your intake forms submitted before we can confirm your first appointment.

Established Patient Intake Form
If it has been more than 1 year since your last visit you will need to complete the Updated Intake Forms. If it has been 3 years or more since your last visit, you will need to complete the New Patient Intake again.

Auto Injury Intake Form
Brand new patients must complete both the New Patient Intake and the Auto Injury Intake. Established patients with a new auto injury must complete the Established Patient Intake and the Auto Injury Form before your first appointment.

Prenatal Care Form
New and Established patients interested in chiropractic or massage therapy during pregnancy need to complete our Pregnancy Intake Forms.

Missed Appointment Policy
ALL established patients need to acknowledge our missed appointment policy by completing this form.

If you have pertinent records from other providers or imaging centers that are relevant to your care with us, please complete this form.
